Location: North Colorado Family Medicine at 1600 23rd Avenue, Greeley, CO 80634.

Schedule: Monday through Friday from 8am to 5pm. No Weekends!

Day to Day: Seeing patients, precepting with residents, teaching residents, academic didactics, general compliance and documentation guidelines, etc.

POSITION SUMMARY

This position is responsible for the development and implementation of the Behavioral Sciences curriculum and the coordination and supervision of the teaching of biopsychosocial issues to resident physicians and medical students in the Family Medicine Residency Program. Participates in the interview, selection and evaluation of residents. Position also provides psychotherapeutic services through group, individual, and family therapy.

CORE FUNCTIONS

  1. Develops, implements and maintains the Behavioral Science curriculum provided to resident physicians and medical students in the Family Medicine Residency Program. Coordinates, supervises the teaching of, and provides education of biopsychosocial issues and topics as related to family medicine per the Residency Review Committee.

  2. Responsible for precepting and supervision of resident physicians and medical students in the Family Medicine Residency Program. Assesses and communicates behavioral health issues and makes recommendations for patient care and the utilization of resources to optimize the education process. Makes rounds with care team in hospital, nursing home, and occasional home visits.

  3. As panel member, participates in the interview, and selection of resident applicants for intern class. Schedules and participates in resident evaluations. Provides input regarding disciplinary action, follow-up and support.

  4. Delivers psychotherapeutic interventions through group, individual and family therapy and education. Provides ongoing assessment of patients' level of care, needs/resources, and recommends interventions based on medical necessity. Develops individual treatment plans.

  5. Provides case management for assigned patients including: coordination of care with treatment providers; monitor utilization of resources and patient progress; educating patients on community resources; and timely and thorough documentation within the patient medical record.

  6. Position is accountable for the development and delivery of behavioral sciences curriculum to provide training and development to resident physicians and medical students. Serves as a member of the interview/selection/evaluation panel. Provides direct psychotherapeutic care and case management to assigned patients. Interacts with residents, medical students, patients, all levels of nursing management and staff, medical staff, and other members of interdisciplinary health care team.

MINIMUM QUALIFICATIONS

Requires a master's degree in social work, counseling, marriage and family therapy, psychology or related behavioral science field.

Requires licensure as marriage and family therapist, professional counselor, or clinical social worker in state of practice.

Requires a proficiency level typically achieved with 5 years experience in behavioral health setting.

Strong organization, facilitation, communication and leadership skills; Requires ability to work with a diverse population along with interpersonal skills and knowledge required in treating patients.

PREFERRED QUALIFICATIONS

Additional certification in behavioral science specialty area preferred.

Additional related education and/or experience preferred.
